LIFE SKILLS CLASS PARTICIPANT WAIVER *

This agreement contains a waiver of claims based on negligence LifeCraft- is dedicated to Life skills education . Our classes are intended for your Child's education and enjoyment. They are conducted under the direction and supervision of professional instructors and may be either demonstration or hands-on classes, at our discretion. Your Child's participation is strictly voluntary. Because culinary activities can present risks, LifeCraft- Shelly Brooks, depends on you to use your good judgment and reasonable care for your Child's own safety and the safety of others. Please follow the directions of the instructor carefully. If you have any questions, whatsoever, about your Child's ability to safely follow any instructions you are given, please immediately alert the instructor. In consideration for your participation in an activity at LifeCraft, and by signing this form, you acknowledge, agree and understand that your Child's participation is voluntary and you assume the risks associated with the activity in which he/ she will be participating. Such risks may include, but are not limited to, the risk of physical injury or harm. You further agree to indemnify, save and hold harmless Life Craft- Shelly Brooks from any and all claims arising out of an injury, disability, or death resulting from your participation in Life Craft classes. This waiver specifically includes any claims made by you arising from the alleged negligent acts of Life Craft, its employees, agents and any related entities.
